A Comparison between Alloderm and Free Gingival Graft

Alloderm and Free gingival grafts are placed on either apical or coronal region at the recessed gingival margin, where these grafts are used to enhance the smile and reduce tooth exposure which gives unpleasant appearance. A free gingival graft is used to increase the zone of attached gingival tissue. In this procedure, the tissue is obtained from the patient itself and used on the site of gum recession. The tissue obtained can be either from the palate of the patient or some other oral tissues from the patient’s mouth. It is then placed on the area where there is exposed root surface or wall defects, and which can have single or multiple defects. There is hardly any damage to the donor due to tissue removal.

The main function of the free gingival graft is to increase the thickness of the gum tissue. This process is indicated when there is gingival recession or a part of the soft tissues is displaced due to trauma. It also covers root surfaces exposed due to severe gum recession. In this technique, an incision is made at the mucogingival junction without touching the periosteum. An Aluminum foil template at the recipient site is used as patterns, and the graft is placed directly on the bone tissue and is allowed to heal for two weeks.

An Alloderm or acellular derma matrix is a processed graft obtained from donated human tissues which is later processed to be used in any patient. Alloderm is made up of a matrix of collagen which helps in cell production and also contains elements which provide blood supply to the surrounding areas. It can be used for root coverage for multiple teeth. And it has very good epithelial tissue which helps in maintaining the esthetics. The cells produce and cause pigmentation like the natural tissues, and give good contour. We can also treat several defects in a single surgery with alloderm graft as there are no limitations on the amount of tissue obtained unlike the free gingival graft obtained from the palate which has limitations.
There are several advantages of the alloderm against free gingival margin graft. Firstly, no palatal tissue is required from the patient. Secondly, multiple surgical procedures can be performed on the patient without any limitation on the tissue requirement. Irrespective of the age of the patient and the medical fitness of the patient, the surgery can be performed as the palatal tissues are not required from the patient. With Alloderm, the height of the mandible over denture is maintained. Alloderm was previously used in major body burns for plastic surgeries, but today it is extensively used in dentistry as well.

Although dental implantology has tremendously advanced in the last decade, there are still a few flaws and backdrops in the procedures when it comes to perfect esthetic appearance, especially for those who want to have natural appearance immediately after implant surgery. Alloderm is a product which is used in human implants gingival grafts, periodontal grafts, gingiva recessions etc etc.

It is made out of real human tissues. Unlike other membranes, Alloderm is natural and they slowly get integrated into the tissue of an individual. Alloderm is used in various oral grafting procedures like augmentation of soft tissue, lip augmentation, gingiva defects around dental implants etc.

All Ceramic Crowns and Esthetic composite are both dental restoration methods. Both are equally popular but they differ widely from each other. This article will give you a brief insight on both these methods.

An all-ceramic crown completely encircles or caps a tooth or a dental implant. Dental cement is used of bonding crown with the tooth.

These are commonly composed of Bis-GMA monomers, a photo initiator and a filler material like silica. The scientific knowledge and artistic ability produce strong, esthetic, and long-lasting restorations. However, this method too has some disadvantages. It is said that composite restoration causes craze lines, stains, dentin exposure, chips, cracks, surface defects, and developmental defects.

However, despite of their various shortcomings both these methods are widely used for dental restoration.

Careful with Internal bleaching

If you have a tooth that had a previous root canal treatment and it's getting discolored, sometimes with a range of colors from grey to brown, sometimes even pink when the dentist left some live pulp inside the tooth, there's a good option to put that tooth with again, and that is Internal Bleaching.

So what do you do?

You drill the tooth through the same it was drilled to do the treatment and you protect it 2mm under the CEJ line with Glass Ionomer. You put the Hidrogen Peroxide in the pulp chamber and you close it for 3 to 5 days. Check the results and repeat if necessary.

So... All seems pretty easy and pretty good, and in fact the tooth get much whiter but the problem is that in clinical trials have showed that this kind of treatment has a recidive rate of aobut 50%, which means that after 3 years half the internal bleachs get back to dark again... Although it's never as the original dark color but it gets darker and you may need to internal bleach the tooth again so...

Should you go for internal bleaching? Or just solve the problem with a Ceramic Crown?

I don't have the answer to that, it depends on many factors, so you should be aware of this before choosing the treatment you want...

First Biohorizons Laser Lok tapered implant

Just got back from the appointment with my first patient receiving a Laser lok Biohorizons Implant to a replace a 13 which had in place the decidous 53, so we made an extraction with imediate placing of the Laser lok 3.8 implant.
I choose the Laser lok in the case because of the gingival retraction the patient had, due to old habit of brushing too hard and the gingival results are great after implant placemente. The gingiva grew a bit, not much, but at least didn't retract anymore.
The implant was also choosed because of the orientation of the apex of the adjacent teeth, which didn't gave me space to place a external conical Biohorizons implant.

For now i'm very happy with Laser Lok, lets see how it osteointegrates...

Regular fillings or Amalgam is made up of silver, zinc, tin, copper and Mercury while ceramic fillings or composite resin is a mixture of fine glass and plastic. Clinical case, Internal bleaching with Opalescence Endo 35%

Here is a clinical case of mine of an internal bleaching of a central incisor 21, using as bleaching agent Opalescence Endo 35%, which active principal is hydrogen peroxide.

The central incisor was really dark, with brown to gray pigmentations and it went from a C3 VITA color to an A1 VITA color in just two weeks time.

This whitening system proved to be very efficient in this case, although it requires RX control every 6 months to prevent external cervical root reabsortion.

But there are any benefits by using a whitening toothpaste?

Tooth paste contains abrasive ingredients that physically with the help of the brush will scrub away plaque. In addition, these abrasives will help to remove food stains from the teeth and polish tooth surfaces.
So until now all good, but there are any inconvenient about these abrasives?
They are responsible by the cleaning power of any toothpaste, they remove the plaque and the foods stains as well as they help to polish the teeth. The most common abrasives are alumina, calcium carbonate and phospates and silica, which gave our toothpaste the feeling that it has sand in it.
But toothpaste manufacturers should balance well these abrasives, it should be abrasive enough to remove plaque and stains BUT not abrasive enough to damage tooth enamel.
Unfortunately that is the bad part, many toothpaste are too abrasive, they do damage tooth enamel and reduce this mineral element of the tooth giving one direct result...Tooth sensivity!
Undirect result can also be yellowing teeth cause the thinned enamel reveals the dentin layer below which is yellow.

So in conclusion, my opinion is:
Do not use a whitening toothpaste!
It will not help you if you have very pigmented teeth, it would only remove small stains. If you really would like to have white teeth, go to your dentist and ask him for the best teeth whitening system for your case.
Use a toothpaste with good % of fluoride and not too abrasive and a good brushing technique, it's the best in my opinion.

What can cause my teeth to get ugly?

So here is a quick resume of what can cause your teeth to be ugly or simply no as white as you would want.
Tetracycline staining which is an antibiotic and shoul be used with careful in young ages especially Excessive fluoride. Some countries have really high % of fluoride in they drinking water and that can cause dental fluorosis which leaves white spots at the teeths
Dental caries...Of course! The acid from the bacterias will desmineralize the tooth and change its colour
Trauma (injury) to teeth can cause a enamel defect and cause spots etc etc
Pulp (nerve) degeneration, necrosis of the teeth can also cause stains
Restorative materials such as amalgam, or old filling can be stained as the time goes by.
Genetic and acquired illnesses, a major variety of them can cause stains at the teeths
Aging, it's normal to our teeth to get darker as we age..
Also as i've mentioned before, changes in the mineral structure, type of food, smoking habits, genetic type of enamel and dentin, low hygiene can cause your teeth to stain.

The dentist should be very careful diagnosing the stain causes, so that he can give the patient the most adequate teeth whitening system for they situation.

Final results with Carbamide peroxide 10%

My patient went back to the clinic and we checked the results.
She's lot better, she had incisors with A3,5 and canines teeth with B3 colour.
After the teeth whitening she is at a A2,5 full colour and it can still be improved.

No damage done to the gum, she just had a bit of sensibility after the bleaching but it's normal.

Teeth whitening is a common practice in general dentistry and it's becoming even more common in the last few years.
Everybody wants a white smile, some people even get paranoid about it. Many
people believe that white teeth are the greatest contribution to
a perfect smile and a perfect smile makes others look at you in a different way for sure. Our smile is like our trademark image, others always get a nice image if we have a nice smile.
As a person gets old, teeth often become darker and stained due to many factors including a change in the mineral structure, type of food, smoking habits, genetic type of enamel and dentin, low hygiene etc etc…

Dentists can choose from a wide range of teeth whitening systems, but what really matters in the end is the percentage of carbamide peroxide in the gel; it varies from 5% to 30%, depending of the manufacter but the only percentage aprooved by the ADA is 10%. Above that there seems to be no evidence that they're totally safe.

Regularly using a whitening toothpaste, which may have a very small amount of peroxide in it, may help keep teeth bright longer but that toothpaste don't often do any whitening by themself, you'll always need some whitening system so that you'll have nice and white teeth.

Whitening systems are almost all the same procedure steps...
You just brush your teeth very well, fill the plastic trays with the whitener gel, and squish them onto your upper and lower teeth and stay with it for a few hours, around 2 to 3 hours.

The resulting whiteness can last for years or months, depending on your lifestyle and how often you brush your teeth, smoking and coffee habits etc etc.

Only about 8-10% of people claim to be satisfied with the colour of their teeth. Routinely eating and drinking foods and liquids that stain tooth enamel will dull the effect more quickly so dentist himself gotta advice the patient if he sees that it's not a good candidate to bleaching products.

If someone has caps on the front teeth, for example, the bleached part of the teeth may turn lighter than the caps and so the dentist must take that in care before giving that kind of patient a bleaching product or else the patient may complian at the end of the bleaching.

I told my patient to use it for 2 weeks and tooks some photos before it, to check the results.
Let's see if the whitening will work, that patient smoked a lot, like 2 packs each day and had not good higiene habits.
